Try enabling fakemeta module.
When your server is up and running, type: amx_plugins in your console. It should tell you what plugins are loading and running. |

The file's likely in the wrong place. Make sure it's the .amxx file and it's in
\ns\addons\amxmodx\plugins |

I would check those forums again. There maybe a revised version of extralevels2 compatable with amxmodx 1.60. I know they were having lots of issues with some plugins, maybe extralevels2_rework was one of them.
